Walking out of surgery with fixed teeth: is it really possible?

One of the questions I hear most often is this: "Doctor, is it true that I can walk in without teeth and walk out the same day with fixed teeth?" The short answer is yes, in many cases you can. We call that technique immediate loading, and it is one of the things that most changes the life of a patient who has spent years hiding their smile.

But I want to be honest with you from the start: immediate loading is not for every case. In this article I explain, without jargon, what it is, how it works, who qualifies and who does not, so you arrive at your evaluation truly understanding your options.

What immediate loading is

When a dental implant is placed, the traditional approach has been to wait 3 to 6 months for the titanium screw to integrate with the bone (a process we call osseointegration) before placing the definitive teeth on top. During that wait, the patient usually wears a removable provisional solution.

Immediate loading changes that sequence. It consists of placing a fixed provisional prosthesis on the implants the same day of surgery, or within the first 24 to 72 hours. In other words: the patient leaves the clinic with fixed teeth that look and feel like their own, while underneath the implants integrate with the bone over the following months.

It is important to understand that those first teeth are provisional. They serve an immediate aesthetic and functional purpose, but the definitive prosthesis, made in zirconia or ceramic, is placed later, once integration is complete.

"Teeth in a day": what it really means

The phrase "teeth in a day" has become popular, and it sometimes creates expectations worth grounding. It does not mean the entire treatment finishes in 24 hours. It means that in a single day you recover a fixed, functional smile, without having to spend months without teeth or with a plate that moves.

The full treatment, up to the definitive prosthesis, still takes several months. What immediate loading saves you is precisely the hardest part to bear: the in-between time without teeth. That is why it is so transformative for those who need it.

Who qualifies for immediate loading in Medellín

Here is the part no advertisement tells you clearly. For immediate loading to work, the implants must be very stable from the very first moment (what we call primary stability). If the implant moves even minimally under chewing forces during the first weeks, integration can fail.

In general terms, you tend to be a good candidate when:

• You have enough good-quality bone for the implants to sit firmly from day one.
• You are rehabilitating a full arch, where several implants splint together and share the forces (the typical case of techniques like All-on-4).
• Your general health is under control and there are no active infections in the area.
• You are willing to follow the first-months instructions to the letter, especially the soft diet.

Who does not qualify (and why that protects you)

Saying "no" in time is also part of doing things well. Immediate loading is usually not the best option when:

• There is severe bone loss and the implants cannot achieve enough stability when placed. In these cases the best route is sometimes to wait for integration, or to consider solutions such as zygomatic implants.
• There is an active infection or untreated periodontal disease.
• The patient has uncontrolled severe bruxism (clenching or grinding with great force), because the overload can compromise newly placed implants.
• There are uncontrolled health conditions that affect healing.

When a case does not qualify for immediate loading, it does not mean you have no solution: it means your solution needs a different sequence. Forcing immediate loading where it is not indicated is precisely what leads to the failures that frighten patients so much.

The only responsible way to know whether you qualify is with a 3D study (CBCT scan) that lets us measure the quantity and quality of your bone before touching anything. With that information we digitally plan the position of each implant and define, with sound judgment, whether immediate loading is viable or another route is better.
